Quincy July 14th: 1799

Sir.

I request you to act as surgeon on board the constitution as you have done from the 17th of December last. I shall write this day to the Secretary of the Navy to send you a commission in form to meet you at Norfolk

I am Sir with great esteem your humble servt.
